If a child or young person has received funding from STAR for an event or activity, we request that an Evaluation Form is completed. Our funders need follow up information as to the impact of their funding, therefore the completion of the Evaluation Form is an essential criteria in order for funding to be granted. It also allows us to learn more about their experience and outcomes.

If you need to complete an Evaluation Form, please click on the button below, fill it in and either email it to or post it to STAR, PO Box 5176, Shepton Mallet, BA4 9DD

“Asked if they would like to have more forest school sessions, all the children agreed that they would and would take part in these.
Thank you, STAR for your support ”
